Retreat Activity Waiver and Media Permission Nervous System Reset Retreat | October 21 to 24 2027 1 Event and scope This form is between Good Vibrations Events LLC (GVE) and the participant identified below. Cosmic Reveil LLC (CR) collaborates in delivery of the retreat. Participants must be at least 18 years old. The retreat takes place in the Phoenix, Arizona area, with guest arrival at 4:00 p.m. on October 21 and departure at 11:00 a.m. on October 24, 2027, Arizona time. Core activities are yoga and sound baths. Possible additional activities include breathwork, meditation, mindfulness, reflection and group discussion, hiking, pool use, cold plunges, and optional hands-on work such as Reiki. Listing an activity does not promise it will be offered or establish that it is suitable for every participant. This form covers only the activities described here, as actually offered and explained. A label such as “ceremonial experience” or “other activities” does not extend consent to an undefined practice. A materially different activity or risk requires advance explanation and a separate informed choice before participation. 2 Your choices and physical contact Every activity is voluntary. You may decline, stop, observe where safe, or ask for a modification without pressure. You do not have to disclose personal experiences in a group. Declining an individual activity is not the same as refusing the required waiver or recording permission. You may inform your facilitator or instructor you do not give affirmative consent for proposed contact such as physical assistance or Reiki-style contact. A no-touch option may always be requested, and you may always choose to skip an activity. 3 Wellness activities are not clinical care GVE and CR are not providing medical services, mental health treatment, psychotherapy, trauma treatment, diagnosis, or clinical care at this retreat. No particular health outcome is promised. Reiki and other practices are not substitutes for appropriate healthcare. Do not delay or stop prescribed care because of a retreat activity. If pregnancy, an existing condition, an injury, medication effects, or uncertainty may affect safe participation, discuss the planned activity with a qualified healthcare professional beforehand. This is not a certification that you are free of medical conditions. Tell the facilitator any limits or modifications needed; this form does not require a diagnosis or complete medical history. 4 Activity specific risks and precautions The following are possible risks, not a claim that every activity causes them or that this list is exhaustive. Safe instruction and precautions reduce some risks but do not eliminate them. Do not continue through pain, breathing difficulty, faintness, confusion, or distress; stop and seek help as appropriate. Yoga and movement Movement, stretching, balance and changes in position can cause strains, sprains, falls or aggravation of an existing injury. Use a level of effort appropriate for you, avoid forcing a posture, and ask about modifications. Sound baths Sound intensity and proximity can be uncomfortable. Sufficiently loud or prolonged sound can damage hearing. You may move farther from instruments, request a lower volume, use suitable hearing protection, or leave. The sound-level and instrument-placement plan must be requested before sessions are offered, and participants may always leave the room. Breathwork Rapid or forceful overbreathing can cause lightheadedness, tingling, weakness, chest discomfort or fainting. The facilitator must explain the actual method, posture, intensity and stop signals before participation; this form does not approve every breathing technique. Breathwork must be kept away from water and situations in which fainting could cause a fall or drowning. Do not combine intentional hyperventilation or prolonged breath-holding with swimming or immersion. Meditation reflection and group discussion These practices may bring up uncomfortable thoughts, anxiety or emotional distress. You may keep your eyes open, pause, leave or decline to share. Group discussion is not clinical care, and group participation cannot guarantee confidentiality. Do not record another guest’s personal disclosures. Hiking Outdoor walking can involve uneven or slippery ground, falls, heat, dehydration, weather changes and delayed access to help. Before opting in, review the route, distance, difficulty, expected conditions and turnaround plan. Stay on the designated route, follow the leader’s instructions and use suitable footwear and supplies. No particular hike, route or transport service is included by this form. Pool use and cold plunges Water activities can cause slips, falls, loss of consciousness, drowning, severe injury or death. Swim with a buddy; follow the site’s access rules and safety briefing, and do not assume a lifeguard is present. Do not dive or engage in dangerous activities. Do not enter while impaired. Cold immersion can trigger involuntary gasping, rapid breathing and sudden cardiovascular stress; it can also impair movement and judgment and cause hypothermia. A short exposure or strong swimming ability does not eliminate these risks. No water temperature or exposure duration is represented here as safe for everyone. Only enter a cold plunge when its approved supervision, entry, exit and emergency arrangements have been explained, and its use by you cleared by a doctor. Optional Reiki style hands on work Reiki may involve hands lightly on or above the body. Contact, personal space and positioning can feel uncomfortable; explain boundaries and stop participation if uncomfortable. It is not medical treatment, and no health benefit is guaranteed. Such contact is optional, should be discussed with facilitator, and participant may choose to skip this activity. 5 Conduct and help in an emergency The retreat is alcohol-free. Follow activity instructions and applicable property rules, respect other guests, and report hazards or injuries promptly. Do not participate while impaired or when medication effects make an activity unsafe. Venue rules remain applicable, but this form does not make a separate venue contract part of your agreement without disclosure. For an apparent emergency, GVE or CR may call emergency services and the emergency contact you provide. Any information shared should be limited to what is reasonably necessary for that response. This is not consent to a particular treatment, an advance directive, or a promise of on-site medical care or immediate rescue. It does not authorize routine sharing of health information for marketing. 6 Assumption of risk and release including ordinary negligence I understand that the activities I voluntarily choose can involve the risks described in section 4, including serious injury and, for certain activities, death. I choose whether to participate after receiving the activity-specific information and an opportunity to ask questions. I accept the inherent risks of those activities to the extent permitted by applicable law. To the extent permitted by Arizona law, I release Good Vibrations Events LLC and Cosmic Reveil LLC, and their owners, managers, members, employees, facilitators and contractors only while acting for this retreat, from my claims arising from the listed activities I voluntarily choose, including claims caused by those released parties’ own ordinary negligence. This release does not apply to claims caused by gross negligence, recklessness, intentional misconduct, or a breach of the express terms of this form, and does not waive rights that cannot lawfully be waived. This form does not require me to indemnify anyone, pay another party’s attorneys’ fees, arbitrate a dispute, or waive a jury. It does not release a venue, an unrelated service provider, or an activity not described here. Any separately required venue form must be provided and reviewed on its own terms. 7 Relationship to your booking agreement The approved Retreat Participation Agreement and your completed Attachment A continue to govern your booking, payments, accommodation, cancellations and refunds. This form does not replace them or create a new refund schedule. GVE requests this separate waiver after your first successful payment and requires it before participation. Your earlier participation-agreement signature did not sign this waiver or accept its future terms. If you expressly decline the required activity waiver, GVE will cancel under the existing cancellation and refund policy. Cancellation takes effect when GVE receives the express decline, using that Arizona calendar date for the refund tier. A waiver not yet completed is not, by itself, a refusal. Declining an individual activity is not an express decline of the waiver. 8 Required photo and video permission Photo and video recording is part of this retreat. I permit GVE and CR, directly or through media vendors acting for them, to capture my image and incidental voice during the retreat and use selected recordings to document the retreat, show their collaboration, and promote their retreat and wellness businesses and future events through their websites, social channels, email and promotional materials. No separate compensation is promised. Recording and use must be respectful and non-invasive. It limits recordings to common and public areas. No filming of private conversations or identifiable personal health disclosures, or recording a person in acute distress will be allowed. Staff must stop an intrusive shot when asked. This safeguard does not create a general no-recording attendance option. I may ask GVE to review a privacy concern or remove a specific item. If I cannot accept the required recording permission, I should contact GVE before signing. The express-refusal and cancellation rules in section 7 apply. This permission is not an email or SMS marketing subscription. 9 Acknowledgments ☐ I am at least 18 years old and have read the approved version of this form. ☐ I understand the listed risks and my right to decline or stop individual activities. ☐ I understand that hands-on contact is optional and that I may decline it, request a no-touch option, or skip the activity. ☐ I have separately read and accept the final approved assumption-of-risk and release language, including its express release of ordinary negligence to the extent permitted by Arizona law. ☐ I accept the required photo and video permission and its stated uses and limits.
